ABSTRACT

Liver fibrosis is the leading cause of the morbidity and mortality of liver diseases worldwide and has become one of the key issues in the research on chronic liver diseases among domestic and foreign scholars. An increasing amount of evidence has shown that gut microbiota dysbiosis plays a crucial role in the development and progression of liver fibrosis, and improvement of gut microbiota dysbiosis has become a new target for anti-fibrotic treatment. At present, traditional Chinese medicine has attracted much attention in improving gut microbiota. Studies have shown that "harmonizing liver and spleen" plays an important role in reducing fibrosis degree and delaying the progression of liver fibrosis. Therefore, based on the theory of "harmonizing liver and spleen", the achievements in liver-gut axis and gut microbiota by modern molecular biological techniques can provide a theoretical basis for the treatment of liver fibrosis from "harmonizing liver and spleen" in clinical practice, as well as new directions and ideas for anti-fibrotic treatment with traditional Chinese medicine.

ABSTRACT

Objective:To explore the status,the risk factors and prevention strategies and measures of cognitive dysfunction in the patients with type 2 diabetes mellitus(T2DM),and the provide the theoretical basis for preventing the mild cognitive impairment(MCI)in the patients with diabetes mellitus. Methods:A total of 192 patients with T2DM who were diagnosed definitely and received hospitalization treatment and accepted the questionnaire willingly were selected as the subjects.The general demographic characteristics,general behavioral characteristics and clinical data were collected.Montreal Cognitive Assessment(MoCA)was used to screen the status of the patients with cognitive impairment,and Activities of Daily Living(ADL)assessment was used to assess the ADL of the patients.Chi-square test,t test and Wilcoxon test were performed to compare the congnitive function between the patients with different features.Multivariable Logistic regression model was used to find the independent influencing factors of MCI in the patients with T2DM.The correlation analysis was used to analyze the relationships between the each module of cognitive function of MoCA and the influencing factors.The P<0.05 was regarded as a statistically significant value. Results:There were 192 patients with T2DM accepted the questionnaire.According to the criteria for cognitive impairment in MoCA,154 of the 192 subjects had MCI,and the incidence of MCI was 80.21%(154/192).Multivariate Logistic regression analysis showed that the elderly and low family per capita monthly income were the risk factors of MCI in the patients with T2DM(P<0.05).The results of correlation analysis between the various modules of MoCA and the influencing factors showed that the age was related strongly to the attention ability(r=-0.334,P<0.05);the family per capita monthly income was strongly related to the visual space ability and executive ability(r=0.322,P<0.05).Conclusion:The incidence of MCI in the patients with T2DM is serious. Age and the per capita monthly income of family are the important factors affecting the cognitive function of the patients with T2DM,which mainly affect the visual space ability, executive ability,attention ability,and memory function.

ABSTRACT

Objective To observe the effect of comprehensive nursing of traditional Chinese medicine and Western Medicine on children′s supracondylar humerus fracture of ulnar deviation on postoperative recovery. Methods From November 2015 to February 2016, 52 inpatients were divided into 2 groups by random digits table method after the external fixation removal surgery with 26 cases each. The observation group was intervened by integrated nursing and was given TCM rehabilitation guidance, while the control group was intervened by regular nursing and was given regular rehabilitation guidance. After 4 weeks, the therapeutic effect was evaluated according to Chinese Medicine Orthopedic Syndrome Diagnosis and their elbow joint function was evaluated by Flynn score. In 8 weeks of follow up, all the patients were valued by their quality of life. Results In the process of intervention, with 1 case fell off in the observation group, 3 cases fell off in the control group, so finally the observation group with 25 cases, the control group with 23 cases. Fifteen cases were cured, 7 cases were improved, 3 cases were not cured at 4 weeks after surgery in the observation group, 8, 6, 9 cases in the control group, and the difference was statistically significant (Z=-2.120, P=0.034). At 4 weeks after surgery, 10, 6, 6, 3 cases were excellent, good, medium and poor evaluated by Flynn score in the observation group, 5, 4, 5, 9 cases in the control group, and the difference was statistically significant (Z=-2.051, P=0.040). Incidence of postoperative complications was 8.0% (2/25) in the observation group, 30.4% (7/23) in the control group, and the difference was statistically significant (χ2=2.622, P=0.047). Physical, psychological, social and environmental fields of quality of life were 77.84 ± 12.15, 81.13 ± 15.72, 79.95 ± 20.68, 79.44 ± 11.57 in the observation group, 63.31 ± 15.86, 63.07 ± 12.63, 65.65 ± 18.51, 61.32 ± 17.24 in the control group, and the difference was statistically significant (t=2.516- 4.363, P<0.05). Conclusions Integrated nursing intervention with TCM rehabilitation guidance is a method which can effectively help in improving the elbow movement function and the life quality of children with supracondylar humerus fractures of ulnar deviation.

ABSTRACT

Objective:To evaluate the reliability and validity of the adaptation OSSIQ in measuring the life quality of the populations with different types of subjective skin.Methods:A stratified samplings strategy was adopted to select 140 college students, workers, personnel of enterprises and institutions aged 18-60 years old.The reliability and validity of adaptation OSSIQ were evealuated using internal consistency validity, split half reliability, and construct validity.Results:The adaptation OSSIQ had a Cronbach's α efficient of 0.924 and 0.902.Split half reliability, the Spearman-Brown before and after skin care were 0.877 and the Guttman Split-Half coefficients were 0.876 and 0.874 respectively.Contentvalidity, the Item-scale correlation coefficients exceed 0.5 (P1.Each factor consisted of the items with factor loading≥0.6;the load factor value belonged to high level (≥0.6) by factor analysis.Conclusion:Adaptation OSSIQ has good reliability and validity for the evaluation on life quality of the populations with different types of subjective skin.It can be used to evaluate the influence of different types of skin in the life quality.

ABSTRACT

Objective To investigate the clinical characteristics and angiographic findings of cardiogenic shock(CS)following acute myocardial infarction(AMI) in elderly patients.Methods Between January 2015 and April 2016,we carried out a retrospective observational analysis of consecutive elderly patients in Tianjin Chest Hospital,who suffered CS-complicating AMI.Emergency angiography and percutaneous coronary intervention(PCI) were performed after admission.All selected patients were divided into CS and non-CS groups according to whether CS occurred.Electrocardiograph (ECG),cardiac enzyme testing,and ultrasound cardiography were performed after admission to monitor the occurrence of CS.Results The incidence of CS-complicating AMI was 8.33% (34/408) in elderly patients.Among all CS patients enrolled,the aged patients accounted for 91.89 % (3 4/3 7).In-hospital mortality rate was 2 9.41 % (10/3 4).There were significant differences between two groups in WBC,H s-CRP,blood glucose,CR and ALT (t =2.403,4.596,6.778,6.109,each P<0.05).The NT-Pro BNP level,the time of FMC,the frequency of left main and multivessel disease were higher in the CS group than in the non-CS group (each P < 0.05).Conclusions Elderly patients are bearing high risk of CS following AMI.Prolonged FMC time and the presence of left main and/or multivessel lesion are independent risk factors for the development of CS.The optimal revascularisation strategy can improve the clinical outcome of patients with CS.

ABSTRACT

The purpose of this study is to investigate the transdermal delivery characteristics of Gentiana macrophylla complex components system through different parts of the skin under micro-needles conditions. Two-chamber diffusion cells were used, different parts of isolated skin and micro-needle pretreated isolated mouse skin were applied separately, high performance liquid chromatography (HPLC) similarity evaluation methods were used to evaluate transdermal delivery characteristics of Gentiana macrophylla complex components system on receiving pool and the permeation rate and penetration amount of Gentiopicroside at different parts of mouse skin. In the 24 h, the similarity between receiving fluid which was on passive transdermal delivery and micro-needle transdermal delivery conditions and original fluid were ranged from 83.0% to 98.9%; By the micro-needle pretreatment with different parts of the mouse skin, the time that Gentiana macrophylla complex components system though abdominal skin to the receiving fluid which reached 90% similarity compared with that of original fluid was 4 h, which was 18 h at back skin and 12 h at neck skin separately. Micro-needles can be used as the ideal ingredients for traditional Chinese medicine complex transdermal delivery; transdermal absorption time delay could be greatly reduced and its bioavailability was improved. The permeation rate and similarity to original liquid of Chinese medicine complex components increased significantly in the abdominal skin relative to the neck and back skin under micro-needle conditions.
